TERMS AND CONDITIONS OF THE LOWER SILESIAN FORUM DOLMED.Tech CONFERENCE – 4th EDITION
§1. General Provisions
1.1. These Terms and Conditions define the rights and obligations of the Conference Participants and the Conference Organisers, hereinafter referred to as the “Terms”.
1.2. The organiser of the Conference is Dolnośląskie Centrum Medyczne DOLMED S.A., with its registered office in Wrocław, ul. Legnicka 40, 53-674 Wrocław, National Court Register (KRS) no. 0000242837, hereinafter referred to as the “Organiser”, in cooperation with UDS Sp. z o.o., based in Wrocław, KRS no. 0000701649.
1.3. The Conference shall be held on the date, at the location, and in the format specified by the Organiser. The Organiser reserves the right to modify these aspects up until the start date of the Conference. The purpose of the Conference is to exchange knowledge and experience among medical and public health professionals and to explore the use of technological solutions in medicine, with particular emphasis on healthcare providers in Lower Silesia.
1.4. Participants of the Conference may include: medical facilities and institutions, companies and startups from the medical sector, government and local administration, institutions and entities offering services to the healthcare sector, university representatives, professionals related to the subject of the Conference, and purchasers of Conference Packages, hereinafter referred to as the “Participants”.
1.5. The Conference may include panel discussions with medical professionals and academic lecturers, Participant presentations, and an exhibition of companies from the medical industry. The current agenda is available at www.dolmed.tech.
§2. Conference Registration
2.1. The conditions for participation in the Conference are:
2.2.1. Submission of a participation request by completing the registration form available on the Conference website or sending the registration form to the Organiser’s address (DCM DOLMED S.A., ul. Legnicka 40, 53-674 Wrocław, e-mail: marketing@dolmed.pl) or registering in person at the Organiser’s Office (as above).
2.2.2. Agreement with the Organiser on the participation terms – package type, scope, and final price, confirmed by the Organiser via email.
2.2.3. Payment of the fee in the amount and by the deadline indicated in the confirmation email referenced in point 2.2.2.
2.3. The deadline for registration via the website www.dolmed.tech is specified by the Organiser on that site. Registrations after the deadline will only be accepted with the Organiser’s consent. The Organiser reserves the right to close registration early if the attendance limit is reached.
2.4. The number of spots at the Conference is limited, including within individual Packages.
2.5. The Organiser is not responsible for incorrect or false information provided by the Participant during registration.
2.6. Submitting the online registration form implies acceptance of these Terms, legal regulations, and any individual agreements made between the Participant and the Organiser. Entities registered before these Terms were published online may withdraw within 3 days of their publication. Failure to submit a withdrawal statement within this time constitutes acceptance.
§3. Packages
3.1. The scope of services provided by the Organiser to the Package purchaser is defined by the package types available at www.dolmed.tech or by individually agreed and confirmed terms via email.
3.2. Accommodation services are provided by subcontractors. The Organiser is not liable for their delivery. Should subcontractors charge the Organiser any penalties or compensation due to the Package purchaser’s actions, the purchaser is obliged to reimburse these within 7 days of notification.
3.3. Company presentations must be submitted for approval to marketing@dolmed.pl at least 14 days before the Conference.
3.4. Opinions and views presented during the Conference are solely those of the speaker; the Organiser is not responsible for their content or form.
3.5. Participants may not express views inciting hatred or discrimination based on race, culture, ethnicity, religion, beliefs, or gender, nor promote ideologies or symbols of totalitarian regimes.
3.6. If a presentation requires third-party permissions or licenses, obtaining them is the Participant’s responsibility. Presentations must comply with Polish law.
3.7. The Participant agrees to make their presentation available to other Conference Participants after the event.
3.8. Technical requirements such as power supply or media access for exhibition stands or presentations must be reported to the Organiser no later than 21 days before the event or the day after registration if within a shorter timeframe. The Organiser will make reasonable efforts to meet these needs and inform the Participant of any issues promptly.
3.9. The Organiser is authorised to take photographs and record video footage during the Conference. Such materials may be used and published for informational and marketing purposes, which Participants accept by agreeing to these Terms.
§4. Fees
4.1. The fee information published in the "Order Package" or "Reserve" sections of www.dolmed.tech does not constitute a commercial offer under the Polish Civil Code. Fees are determined individually after registration as per §2.2.
4.2. Conference fees must be paid to the account specified in the pro forma invoice.
4.3. Failure to make payment by the date stated in the pro forma invoice entitles the Organiser to cancel the registration without liability. The registrant will be notified within 2 business days.
4.4. Proper invoices will be issued after payment.
4.5. Pro forma and VAT invoices may be issued by UDS Sp. z o.o. or DCM Dolmed S.A.
§5. Withdrawal from Participation
5.1. In case of withdrawal or absence after payment, the fee is non-refundable. If the Participant has not yet paid, they are still obligated to do so. Withdrawal or non-attendance does not constitute contract termination and does not release the Participant from payment obligations.
§6. Complaints
6.1. Complaints must be submitted in writing by registered mail with acknowledgment of receipt to the Organiser’s address or via email to marketing@dolmed.pl.
6.2. Complaints must be submitted no later than 3 days after the Conference ends.
6.3. Complaints submitted after this deadline will not be considered.
§7. Personal Data
According to Article 13 of the General Data Protection Regulation (GDPR) of April 27, 2016 (EU Official Journal L 119 of 04.05.2016):
7.1. The Data Controller is DCM DOLMED S.A., ul. Legnicka 40, Wrocław.
7.2. The Controller ensures that your data is handled securely and legally.

7.4. Your personal data will be processed:
a) under Article 6(1)(b) GDPR – for contract execution,
b) under Article 6(1)(c) GDPR – to fulfill legal obligations including documentation and invoicing,
c) under Article 6(1)(f) GDPR – for direct marketing purposes,
d) under Article 6(1)(a) GDPR – if consent is granted, for sending commercial information.
7.5. Your data may be shared with third parties supporting the Organiser, including accommodation, catering, IT services, and other service providers.
7.6. You have the right to access, correct, delete, limit processing, and transfer your data. You may withdraw consent at any time by contacting the Controller or Data Protection Officer.
7.7. You may object to processing not based on consent, contract, or legal obligation, especially for direct marketing. If such an objection is upheld, processing must cease unless there are overriding legal grounds.
7.8. Data will be stored for up to 10 years or until a valid objection is filed.
7.9. You may lodge a complaint with the President of the Personal Data Protection Office (UODO).
7.10. Providing data is voluntary, but refusal may result in non-fulfillment of services.
§8. Final Provisions
8.1. If the Conference is canceled due to reasons solely attributable to the Organisers, paid fees will be refunded to the Participants’ bank accounts.
8.2. Disputes shall be settled by a court with jurisdiction over the Organiser’s registered office, unless mandatory law provides otherwise.
8.3. The Conference is a closed event and does not constitute a mass event under the Polish Act of March 23, 2009 on the safety of mass events.
8.4. In matters not covered by these Terms, the provisions of the Civil Code apply.
8.5. The Organisers are not responsible for lost, damaged, or stolen items during the Conference. Participants bear full liability for any damages caused by them at the venue or accommodation facilities.
8.6. These Terms come into effect on the date of their publication on the Conference website.
8.7. The Organiser reserves the right to amend these Terms. Amendments come into force 3 days after publication unless otherwise stated. For services already underway, the version in force at the time of booking applies.
8.8. These Terms are an integral part of the Conference registration or Package purchase and are binding for all Participants.
